Responsibilities:
- Facilitates the admission and discharge of members.
- Complies with DTA schedule, rules, policies, and procedures.
- Assists in the self-administration of medication.
- Responsible for conducting community meetings and groups.
- Responsible for implementing daily activity and DTA schedules.
- Transport members to scheduled program activities/events, outings, local agencies as deemed on their treatment plan and when applicable unscheduled activities/events.
- Responsible for the safety of the members
- Responsible for documenting daily progress notes when applicable.
- Responsible for protecting member’s rights, confidentiality of the member and medical record.
- Notify supervisor in the event of a safety and clinical issue requiring immediate attention.
- Responsible to ensure members follow their daily schedule and when indicated with their daily living tasks.
- Assisting members in exercise/wellness activities such as walks in the neighborhood.
- Assisting members in daily living skills when applicable
- Knowledge of First Aid & CPR and continual training
- Responsible for implementing the Safety/Disaster Plan when applicable.
- Responsible for completing and incident and Accident report when indicated.
- Responsible for successfully completing job specific and annual trainings as indicated in the training plan.
- Responsible for abiding by the Code of Ethics, conduct business in a professional manner.
- Responsible for providing at least two weeks’ notice requesting vacation time.
- Responsible for calling in sick 8 hours prior to scheduled time to work.
- Responsible for working out schedule changes.
- Other duties as assigned.
Requirements
- Must have or must be able to obtain a level 1 fingerprint clearance card.
- One or more years of experience in this position or related field recommended however willing to train.
- Driver’s license Recommended but not required.
- Must be at least 18 years of age or older.
- Ability to pass a background check and drug test where applicable.
- Ability to speak, read and write English proficiently.
- Great communication skills, maintain professionalism and is punctual.
- Ability to learn prevention and support methods and implement them.
- Ability to learn how to administer medication.
- Ability to pass all required training Ability to lift 50lbs and perform physical activities.
