What is Residential Habilitation?

Funded and supported by the Arizona Department of Developmental Disability. We are an organization created to assist people with disabilities who need special care and support. We offer eight residential group homes that provide 24/7 care and services to our members, with environments that fit and meet different disability needs. Our organization provides total care services, including support for people with a demonstrated need for continuous training, assistance and supervision. We provide protective oversight and supervision to assist with the acquisition, retention or improvement in skills related to living in the community.

How do we accomplish this service?

We accomplish this by collaborating with each member’s designated team and review their Person-Centered Service Plan (PCSP). We assist members in advocating for themselves during CFT meetings, contribute to the process of assessment, ongoing evaluation and communicate member needs to their support system and care coordinators. We contribute by offering observational insight, tracking behaviors and progress and prioritizing each member’s specific needs.

We manage/support member’s medical and mental health treatment and assist guardians in this process. This includes scheduling and attending all necessary health related appointments with the members.

We assist with the following daily living skills:

  • toileting
  • feeding
  • nutritional assistance
  • personal hygiene
  • monitoring health and physical conditions
  • assistance with the administration of medication
  • communicational and interpersonal skills
  • household chores
  • mobility
  • financial management
  • motor and perceptual skills
  • problem solving
  • decision making skills
  • educational needs
  • occupational needs
  • community involvement
  • transportation
  • encouragement of social, recreational and cultural activities utilizing community supports
